2022-2023 Final Forms due Friday, August 19

For students to receive a school-issued Chromebook for the 2022-2023 school year, parents/guardians of every student enrolled at Westfall Local Schools will need to complete Final Forms online registration before AUGUST 19. The forms can be found on our website or by clicking  

Westfall Local Schools has partnered with Final Forms, an online forms and data management service, to simplify the back-to-school process for our families while reducing our paper usage. Final Forms allow you to complete and sign your student's enrollment, back-to-school, and athletic/club participation forms.

Final Forms saves data from season to season and year to year, meaning that you will never need to enter the same information twice! Final Forms also pre-populates information wherever possible for each student, saving you time.

You may review your data at any time throughout the school year to verify that it is current. However, you will be required to sign your forms once yearly, after any update, or as additional documents are added. For grades 5-12, some forms will require a student's signature. Instructions for Parents and Students to access their Final Forms accounts are below.

Forms Due by August 19, 2022

While the initial registration may take 15-20 minutes, it is MANDATORY. Individual accounts "follow" students throughout their Westfall career, requiring only relevant updates from year to year. Therefore, the first registration requires the most time. 


If you are currently a Westfall parent with a Final Forms account: Please login into your parent account and complete the 2022-2023 school forms. Parents must complete each section in the menu on the left side of the page and "electronically sign" (type your name) where appropriate.


To access your Final Forms account:

After your Westfall email account has been activated, go to Final Forms, click Login under Student and then click "Never logged in?" Next, provide your Westfall student email address and click Send Secure Link. Final Forms will send a confirmation email to your student email account. A student can only sign back-to-school forms if the parent/guardian has completed their required forms.

Change of Address: To be eligible for district transportation, Westfall Local Schools must have the student's current address on file. If you have recently moved, please update the student's Final Form with the current address and mail or drop off two (2) proof of residency documents to confirm the address change to your student's building.

Required documents can be a copy of a rental/lease agreement, mortgage coupon, property tax statement, a current utility bill, or an Ohio Driver's License with a current address.

Custody Agreement Updates: If there is a new court-ordered shared custody agreement, please provide a copy of this documentation to your student's school at your earliest convenience.
